Who is the assessment for?
Stakhovsky Tennis uses the assessment with adults and competitive juniors. Leonard Stakhovsky adapts the drills and match questions to the player. It can help a club player returning to tennis, a junior whose progress has slowed or a visitor planning a short Prague training block.
New players can enquire; tell the coach you are new to tennis. A local group class may be a better first step if you want to learn the basics and meet other players. This is a coaching review, not a health check. It does not promise a place on the junior roster.

Where does the assessment take place?
Leonard Stakhovsky coaches for Stakhovsky Tennis at TK Spoje, Hodkovičky and CLTK in Prague. The court, surface and time are confirmed for your booking. Sessions are available in English, Czech and Ukrainian.
Bring your racket and shoes for the booked court. You can share a match-video link after contact. Show full points, not just your best shots. Do not send health records or ID through the form.
Assessment questions
Is the assessment fee refundable if I do not continue?
The Stakhovsky Tennis assessment fee pays for the 90-minute session with Leonard Stakhovsky and the written summary. It is credited in full toward your first program period if you continue. The credit is not a promise of a refund if you decide not to train further.
Do I pay when I send this form?
No. The form sends an enquiry to Stakhovsky Tennis. Leonard Stakhovsky confirms availability, court costs and booking details before a session is agreed. Sending it does not reserve a time or require payment.
Can a parent watch a junior assessment?
Ask Stakhovsky Tennis when you enquire. Leonard Stakhovsky will check the rules for the booked court. Do not assume that a parent can enter every court area. Care outside the agreed session is not part of this service.
Do I have to commit to a monthly program?
No. You can book the Stakhovsky Tennis Performance Assessment with Leonard Stakhovsky as a standalone review. A longer program is discussed only after the findings and your schedule are clear.
